Deadline for Insurance Declination (Sell back) Paperwork has to be received at Central Office Fri Dec 7th. It must be in no later than 4PM. The Open Enrollment Deadline is Friday, Nov 30th. Contact Trish at Central Office 631-345-2195 for more information.

AFLAC's enrollment deadline is Friday Dec 7th in order to be enrolled for 2019. AFLAC will be at MITA on Tuesday November 27th from 2:00PM - 5PM for any members who have questions, need to enroll, or meet with an ALFAC representative.
